Medicaid Service Coordination
The Arc of Rensselaer County’s Medicaid Service Coordination team is here to support individuals and families who need to transition from a state operated (OPWDD) Medicaid Service Coordinator to a voluntary provider. Our service coordinators partner with individuals through the ISP (Individualized Service Plan) process to identify an individual’s valued outcomes and by accessing the appropriate community services. Integrated in our approach to service provision is the philosophy of providing informed choice, person centered services, relationship building and being creative in our efforts to support people in achieving their life goals.
Specifically, a Medicaid Service Coordinator facilitates access to the following services:
• Benefits (i.e., Food Stamps/Medicaid/Social Security/DSS)
• Referrals for new living situations
• Referrals for community programs (i.e., Supported Employment/Day Habilitation/Recreation)
• Support with Educational Services (i.e., IEP meetings)
• Trusts/Burial Funds
• Respite Services
